A Foundations Initiative

Program Overview

The Options Niagara program is designed to assist young adults with a developmental disability acquire the skills necessary to make a transition from school to life as mature citizens in the community. Options Niagara is a person-centered program that utilizes a holistic approach with an emphasis on personal development and improving quality of life. The program provides a connection to short-term outcome-based supports in all aspects of community life.

Who can participate?

Young adults exiting high school with a developmental challenge. Each participant will receive assistance setting their personal goals and accessing community supports and services.

Options Niagara Procedures

Options Niagara uses a two-phase process to assist individuals in identifying goals and accessing supports and services.

Phase 1:
Each participant will meet with a Program Facilitator to complete a vocational, life skill and quality of life assessment. This will lead the individual to define their strengths and plan for the future. The participant and the Program Facilitator will then develop an action plan.

Phase 2:
Phase 2 of the process provides assistance to participants who require short-term, specialized support to achieve greater independence but whose needs may not be met through existing services. The specialized supports can assist in building greater independence in Vocational and Community Life Skills.

Who is Eligible?

Individuals must...

  • Have a developmental disability.
  • Be between the ages of 18 and 28.
  • Have completed or be in their last semester of high school.
  • Have the ability to participate meaningfully in identifying their goals.
  • Be motivated to work toward greater independence and skill development.
